If you are interested in ministry opportunities within the Rocky Mountain CBA region, or one of the other CBAmerica regions, you can download the Word file or .pdf form from this page. Once you have filled out the form you will need to save the document.After completing the form you then email the completed document as an attachment or print and mail the form to our offices.

There is a $50.00 fee that needs to be sent to our offices prior to our giving the form to any church for consideration. We will send the completed form on to any other region that you have indicated. That region will not assess any additional fees.
